$110 per 50-minute session

Insurance
Mental Health services may be covered in full or in part by your health insurance or employee benefit plan.  I am a contracted provider for Managed Health Network, United Behavioral Health, Pacificare, Anthem Blue Cross, Blue Shield of California, and Aetna Behavioral Health among others.  Your health insurance plan might cover my services even though it has another name.  Please check your coverage carefully by asking the following questions:

• Do I have mental health insurance benefits?
• What is the coverage amount per therapy session?
• What is my deductible and has it been met?
• Am I allowed to utilize the services of an out-of-network provider?
• How many sessions per year does my health insurance cover?
• Is approval required from my primary care physician?
• Do I need an authorization before utilizing services?

Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified me at least 24 hours in advance, you may be required to pay the full cost of the session.
